Please check off each of the items below to indicate that your organization agrees to comply with the contract item

 1.

The documentary will be shown at the dates indicated above. The host organization will not have private screenings nor will the host organization or any of their guests make any copies of the DVD.

 2.

The Yoga Unveiled production company will mail out two copies of the DVD two weeks prior to the premiere weekend. Instructions for submitting payments and how to mail back the DVDs will be included in this package.

 3.

The price per attendee per ticket is $10.00 USD. (Canadian hosts charge a minimum of $12 CAD.).

 4.

The host organization will pay the Yoga Unveiled production company $5.00 USD per ticket sold (50% of $10.00 USD ticket price) each time the documentary is shown on the World Premiere weekend.

Example:

• Two screenings x 100 attendees each = 200 attendees x $10 per ticket = $2,000 USD revenue received by host organization.
• $2,000 revenue received x 50% to Yoga Unveiled = $1,000 payment due Yoga Unveiled

 5.

The minimum amount to be paid to the Yoga Unveiled production company is $250.00 USD for the Yoga Unveiled World Premiere Weekend (i.e. 50 attendees).

 6.

The host organization will be responsible for all costs associated with the World Premiere events (equipment, comp tickets to volunteers, advertising, tax, etc.).

 7.

Payment, Post Event Information Form and any outstanding copies of the DVD for the Yoga Unveiled World Premiere are due back two weeks from the date of your last contracted public screening. The organization will obtain a tracking number from the courier or postal service to ensure delivery of payment and the DVDs.

 8.

Payment will be recieved after two weeks of the screening date by one of the following payment methods: Check, Money Order or Credit Card:

• Credit card payment will be accepted in the following manner by Fax: 203-230-4656 or by Telephone: 860-232-3331.

A late fee of $25.00 USD will be charged if payment is not received by this date.

 9.

For our international host sites, all payments to the Yoga Unveiled production company will be made in U.S. Dollars only.

 10.

Prior to the week before the event, a $75 cancellation fee will be charged to the Host Site for removal of their information from www.yogaunveiled.com. Any cancellations within a week of the showing will be charged the $250.00 minimum.

 11.

Yes, we are interested in learning more about the opportunity to enhance our revenue by selling the 2 part DVD set of Yoga Unveiled at our screenings. Additional details will be sent to the host organization with the confirmation host letter.
